Single View of Patient Care Pathways (PCP)
Reducing variation to unlock value
For the foreseeable future the National Health Service must rise to the challenge of providing first rate patient outcomes and clinical effectiveness while operating with constrained resources. Between 2011 and 2014 the NHS needs to find £20 bn in efficiency savings while responding to Government plans to achieve a more patient centric health service.
The NHS atlas of variation shows for the second year in a row significant unjustified variation in care and healthcare spending between regions. With spending on areas of care such as mental health, diabetes, dementia and some cancers showing as much as two fold variation in spending, there has never been a more important time to improve care pathway management.
The Entity Single View of Patient Care Pathway service (PCP) enables a healthcare organisation to efficiently manage the pathway of care for patients, once they have been
diagnosed and the care pathway identified. The objective of the service is to both save significant cost (millions of pounds for a trust for each care pathway developed) and to improve the quality of care that is provided.
